Living with a heart condition: Social care: we need your views
Social care is the support that many people with long term conditions, people with disabilities, older people and carers use every day.
Depending on a person’s needs, it can mean help to dress, wash, eat, communicate or get out and about. It can mean someone coming into your home for just 1 hour per week, going to a drop in day centre to receive help with communication or 24 hour support provided in a residential care home.
I’m trying to find out what kinds of social care people with cardiovascular disease would benefit from and what they are currently getting.
To help me find out more, if you have, are a family member of, or care for someone with cardiovascular disease it would be really useful if you could let me know answers to these questions:

- What kinds of social care support do you currently get?
- What has been your experience of trying to get this support?
